What is a Medical Home?

A Medical Home is the team you choose to provide health care services for your child. A Medical Home team is a group of health care professionals who provide health care services and get to know you and your child’s health care needs. When you and the Medical Home team work together, your child gets the best care possible.

Department of Health Care Policy and Financing

The Department of Health Care Policy and Financing administers the Medicaid and Child Health Plan Plus programs as well as a variety of other programs for Colorado's
low-income families, the elderly and persons with disabilities.
